- New Patient Forms — If you spoke with us on the phone regarding your first appointment, we probably recommended that you show up 10-15 minutes early for your first appointment. This is because there are a couple short forms to fill out before you get started. To streamline the process even further, you can choose to print out the forms here and save yourself the time.
- Insurance List — One of the most common questions we get is: “Does your practice accept my insurance?” We accept most major insurances, including most of the AHCCCS plans. Please take a look at this list, and give us a call if you have any questions.
